Below you will find your Estate Planning Organizer we use in the planning process. The document is in pdf format and can be filled out on your computer and returned to me by email or printed and filled out offline and sent to me by mail or fax.

Please return the completed Estate Planning Organizer at least 24 hours in advance of our meeting. Providing this important information will allow us to use our time together efficiently and focus on those concerns and questions most important to you.
